Cancellation/Late Policy
If there is a need to reschedule or cancel an appointment, please do so by phone at least one business day in advance of appointment time. We recommend arriving to appointments early because if you arrive more than 15 minutes late, you may be asked to reschedule. Starting now, a fee will be applied for missed appointments. The fee is $25 for the first occurrence, $50 for the second, and unfortunately, after the third, we won't be able to reschedule appointments. We appreciate your understanding and cooperation. Payment Policy
For payment and co-payments, we accept cash, checks, VISA, MasterCard, Discover and American Express. All payments are due when services are rendered.
Medical Records
All medical record requests must be inquired on a HIPAA approved form. Please allow at least five business days for processing all forms. Medical records released to a new provider or specialists will be forwarded at no charge. Records released to the patient, certain insurance companies, law firms, or other requests may be subject to copying fees.
