Preparation For Your First Appointment
When preparing for your first appointment at Kinston Regional Pain Center, you must arrange for our physicians to have a copy of your medical records. Based on the health system, it may be required for you to sign a “Release of Medical Records” request form. This should be prepared at least two weeks prior to your first appointment.
Although medical records are helpful, they do not provide all the information needed for your physician to most accurately treat your condition. In addition to providing your records, we recommend that you compose a list of information including:
- Current health conditions
- Any medications that you are currently taking, both prescription and over the counter
- Allergies
- Complete health history, including previous health conditions you have had, previous medications you have taken, and procedures you have undergone
- Family health history
- Name and contact information of previous doctors you have seen
It is also helpful to keep record of your pain through a pain diary outlining when your problem started, your signs and symptoms, the severity of the pain and any self-treatments or medications you have used for the problem. This diary may also include any answered you may want answered during your appointment. The more specific your health history, the more accurately we can diagnose and find the best solution to treat the problem.
We ask that you arrive 15 minutes early to your appointment to allow enough time to complete our paperwork. Please bring any medications, your insurance card, photo identification, and any medical documentation done by another doctor about your area of pain to your appointment.
